Norfloxacin is classified as a quinolone antibiotic. It works by stopping the growth of bacteria. Norfloxacin shows broad-spectrum action against gram-positive and gram-negative bacteria.

Uses
What is Norfloxacin used for?
It is used to treat some types of bacterial infections, such as:
- complicated upper and lower urinary tract infections including cystitis, pyelitis, cystopyelitis, and pyelonephritis
- some sexually transmitted infections
- uncomplicated urethral and cervical gonorrhea due to Neisseria gonorrhoeae
How should Norfloxacin Tablets be used?
This medication is intended to be taken orally. The tablets should be taken with a glass of water. Norfloxacin Tablets should be taken at least 1 hour before or 2 hours after food, milk, or dairy products. It is also important to take them at least 2 hours before or 2 hours after any product containing iron, zinc, aluminum, or magnesium, such as antacids, mineral supplements, or multivitamins.
What dose should be taken?
Recommended dosage of Norfloxacin may vary based on different medical conditions and age.
Adult Dosing Recommended dosage may vary based on different medical conditions:
- For susceptible infections, the usual dose is 400mg, taken every 12 hours. The duration of treatment is 3-21 days. The duration depends on the infection type and severity of the illness.
- For uncomplicated urinary tract infections (UTIs) or cystitis due to Escherichia coli, Klebsiella pneumoniae, or Proteus mirabilis, the usual dose is 400mg taken twice per day. Treatment should continue for 3 days. The maximum dosage is 800mg per day.
- For uncomplicated UTIs due to other organisms, the usual dose is 200-400mg taken twice per day. Treatment should continue for 7-10 days. The maximum dosage is 800mg per day. This drug is effective against susceptible strains of Enterococcus faecalis, E coli, K pneumoniae, P mirabilis, Pseudomonas aeruginosa, Staphylococcus epidermidis, S saprophyticus, Citrobacter freundii, Enterobacter aerogenes, E cloacae, P vulgaris, S aureus, or Streptococcus agalactiae.
- For complicated urinary tract infections, the usual dose is 400mg taken twice per day. Treatment should continue for 7-10 days. The maximum dosage is 800mg per day. This treatment plan should be used for the susceptible strains of the following microorganisms: E faecalis, E coli, K pneumoniae, P mirabilis, P aeruginosa, or Serratia marcescens.
- For acute or chronic bacterial prostatitis due to E coli, the usual dose is 400mg taken twice per day. The duration of therapy should be 28 days. The maximum dosage is 800mg per day.
- For uncomplicated gonorrhea, the usual dose is 800mg taken as a single dose. It is recommended to consult local guidelines, as there are high levels of resistance to fluoroquinolones in some regions of the world.
- For pyelonephritis (mild), the usual dose is 400mg taken every 12 hours. Treatment should last for 14 days.
The exact dosage is based on medical condition, response to treatment, age, and weight. Refer to a doctor or pharmacist for guidelines on dosage. Do not exceed what they advise.
Renal Dosing For patients with CrCl < 30, the adjusted dose for UTIs is 400mg taken once a day. Guidelines have not been defined for other types of infections.
Hepatic Dosing Dosing adjustment guidelines have not been defined for patients with hepatic impairment.
Who can use Norfloxacin?
Norfloxacin Tablets can be given to adults and children, but caution is advised for specific groups of patients.
Pregnant This medication is not recommended for use by pregnant women and is contraindicated in some regions. Norfloxacin should only be used if the benefits outweigh the risks. Animal studies have shown the potential for possible fetal harm, but limited human data has not shown any increase in harmful effects.
Breastfeeding This medicine should only be used if the benefits outweigh the risks. Its use is contraindicated by some authorities. If there is a clear need for the drug, it is advised to wait 24-48 hours to breastfeed. A low amount of Norfloxacin is passed in milk. It is important to monitor for gastrointestinal problems in the infant, such as diarrhea or candidiasis.
Children The safety and efficacy of this medication have not been established for pediatric patients younger than 18 years old.
Geriatric Norfloxacin can be used for elderly patients with no special consideration. It is important to note that renal impairment is more prevalent among this population, so adjustments may be necessary for some older patients.
How should Norfloxacin Tablets be stored?
It is recommended to keep the tablets in their original packaging until use. The medicines should be in a location that is 25°C. The drug can withstand short excursions to 15-20°C, but it should be protected from any extreme heat or cold.
Other warnings
Fluoroquinolones have been associated with severe and potentially irreversible adverse effects, such as tendinitis, tendon rupture, peripheral neuropathy, and central nervous system (CNS) effects.
The increased risk of tendinitis and tendon rupture affects all ages but is further increased for patients who are older than 60 years old, also taking corticosteroids, or those who have had a liver, heart, or lung transplant. Additional increased risk factors include excessive physical activity, renal failure, or tendon disorders like rheumatoid arthritis. It is important to note that this drug class has caused tendinitis and tendon rupture in patients without these risk factors.
Peripheral neuropathy has been reported by patients taking Norfloxacin or other fluoroquinolones. Symptoms typically occur shortly after starting treatment with this drug, and it is important to note that it may be irreversible in certain patients. For patients with a history of peripheral neuropathy, this drug should be avoided.
This drug should be used cautiously in patients who have a known or suspected CNS disorder, including cerebral arteriosclerosis, epilepsy, or a history of seizures.
Norfloxacin should be avoided for patients with a known history of myasthenia gravis, as deaths have been associated with this use of this medication.
Side Effects
As with all pharmaceuticals, some unwanted effects can occur from the use of Norfloxacin Tablets.
Common side effects include, but may not be limited to:
- nausea
- heartburn
- mild diarrhea
- stomach cramps
- dizziness
- headache
- difficulty sleeping
- sun sensitivity
Stop taking this medicine and call a doctor if the following occurs:
- signs of a tendon rupture, such as sudden pain, swelling, bruising, or movement problems in the joints
- nerve problems, such as numbness, tingling, or increased sensitivity to temperatures, light, or touch
- signs of an allergic reaction, such as skin rash, difficulty breathing, or hives
- headache with chest pain, dizziness, fainting
- jaundice, dark urine, or clay-colored stools
- muscle weakness
- watery or bloody diarrhea
- mental changes, such as depression, confusion, hallucinations, paranoia
- seizures
- increased pressure in the skull

Precautions
Do NOT use Norfloxacin Tablets if:
- You are allergic to any of the ingredients.
- You are taking certain antiarrhythmic medicines.
- You have a history of ruptured tendons caused by norfloxacin or another fluoroquinolone.
Due to possible drug interactions, consult with your doctor about any medications, supplements, or herbal products you are taking before your treatment. Norfloxacin is known to interact with drugs such as theophylline and diuretics. It is also known to interact with caffeine, multivitamins, and antacids. This is not a complete list.
This medication may not be suitable for people with certain conditions, so it is important to consult with a doctor if you have any health conditions. Special considerations may be necessary for certain conditions, such as:
- muscle disorder
- history of joint or tendon problems
- inflammation in joints and tendons
- history of nerve problems
- seizure disorder
- Marfan syndrome
- Ehlers-Danlos syndrome
- blood vessel problems
- hypertension
- kidney, heart, or lung transplant
- kidney disease
- heart problems or a family history of heart problems
- diabetes
- colitis
- dehydration or crystalluria
Sun sensitivity may occur while taking this medicine. It is recommended to avoid sunlight or UV light exposure while undergoing treatment with norfloxacin.
Some people experience dizziness after taking norfloxacin. It is advised to avoid driving or operating machinery until the effects of the medication are clear.
It is recommended to avoid drinking alcohol. The combination of alcohol and norfloxacin may increase the severity of adverse effects.
This drug may interfere with certain live bacterial vaccines (e.g. typhoid vaccine). Consult a healthcare profession if you have an upcoming immunization or vaccination.

Review of the Efficacy of Oral Norfloxacin in Pyelonephritis and Nosocomial Urinary Tract Infection
This study included one hundred thirty-eight patients diagnosed with pyelonephritis who received treatment with norfloxacin at a dose of 400mg twice daily. Among them, women constituted 74% of the cases, with Escherichia coli being the most commonly isolated pathogen, accounting for 51% of the cases.
Antibody-coated bacteria (ACB) tests were conducted in 48% of the patients, and of those tested, 72% (48 out of 67) were found to be positive. Forty percent of the patients presented with temperatures exceeding 37.6 °C at the onset of the study. Cure rates among patients with both fever and positive ACB tests were comparable to those of afebrile patients who tested negative for ACB.
Norfloxacin had high effectiveness in treating multidrug-resistant, nosocomial urinary tract infections (UTIs), where Pseudomonas aeruginosa and E. coli were the predominant pathogens.
The conclusion of this study is that norfloxacin is a highly effective alternative therapy for certain UTIs.
