General Information:

Benzyl Penicillin Sodium Powder of Injection is narrow spectrum antibiotic used to treat infections caused by susceptible bacteria. It is a natural penicillin antibiotic that is administered intravenously or intramuscularly due to poor oral absorption. Penicillin G may also be used in some cases as prophylaxis against susceptible organisms.
Natural penicillins are considered the drugs of choice for several infections caused by susceptible gram-positive aerobic organisms, such as Streptococcus pneumoniae, groups A, B, C and G streptococci, nonenterococcal group D streptococci, viridans group streptococci, and non-penicillinase producing staphylococcus. Aminoglycosides may be added for synergy against group B streptococcus (S. agalactiae), S. viridans, and Enterococcus faecalis.
The natural penicillins may also be used as first or second line agents against susceptible gram-positive aerobic bacilli such as Bacillus anthracis, Corynebacterium diphtheriae, and Erysipelothrix rhusiopathiae. Natural penicillins have limited activity against gram-negative organisms; however, they may be used in some cases to treat infections caused by Neisseria meningitidis and Pasteurella. They are not generally used to treat anaerobic infections. Resistance patterns, susceptibility and treatment guidelines vary across regions.

Common side effects include: severe stomach pain, diarrhea that is watery or bloody; fever, chills, swollen glands, skin sores, muscle pain, feeling short of breath, warmth or redness under your skin, severe dizziness; white patches or sores inside your mouth or on your lips; easy bruising, skin rash, unusual bleeding, pale or yellowed skin, severe tingling, numbness, muscle weakness; red or pink urine, dark colored urine; swelling in your hands or feet; muscle twitching, seizures (convulsions); or signs of an electrolyte imbalance–dry mouth, increased thirst, mood changes, confusion, stomach pain, vomiting, muscle pain or weakness, lack of energy, irregular heartbeats, dark urine.
Common side effects may include:
- Mild diarrhea;
- Nausea, vomiting;
- Black or hairy tongue; or
- Pain, swelling, bruising, or irritation around the IV needle.

Precautions
Do not take benzyl penicillin sodium powder for injection if:
- You are hypersensitive to any of the ingredients;
- You have hypersensitivity towards penicillin ;
- You are diabetic or have kidney problems;
- You have a medical history of heart problems.
Studies have shown that administration of benzyl penicillin sodium powder for injections in pregnancy or in women of childbearing age is safe and does not lead to direct or indirect harmful effects to the unborn baby Although it is not known if benzyl penicillin sodium may be excreted into the breast milk of nursing mothers, it is actively transported from the blood to milk in animals and trace amounts of other penicillin in human milk have been detected.

Substantially high doses of benzyl penicillin sodium can sometimes lead to hypokalaemia (low concentration of blood in the potassium) and hypernatraemia (high concentration of sodium in blood). To prevent from these conditions, a diuretic is always recommended. Electrolyte balance, blood count, and renal function should be monitored if the undergoing high-dosage treatment is for more than 5 days.
In diabetic patients due to high blood glucose levels, delayed intramuscular absorption may be seen.
Overgrowth of non-susceptible organisms or yeast may result from prolonged use benzyl penicillin which has to be carefully monitored in patients for other bigger infections.
Administration and Dosage
Administration of dosage can be given intravenously and intramuscularly.
Refer to your doctor or pharmacist for guidelines on dosage. Do not exceed the recommended dosage. Recommendations of dosage are based on the severity of the infections, age, gender and bodyweight.
Intravenous injection or infusion is recommended whenever there is a requirement of high dosage. To avoid irritation of the central nervous system or electrolyte imbalance, slow delivery of injection should be given; taking at least one minute for each 300 mg (0.5 mega units). Hypernatremia and hypokalaemia are seen when injected with high doses unless the sodium content is taken into account.
Interactions
Benzyl penicillin sodium powder for injection interacts with other drugs such as: oral contraceptives, probenecid, methotrexate, allopurinol or sulfipyrazone, neomycin and anti-coagulants.

Overdose
Symptoms of overdose include trouble breathing and passing out. These symptoms have to be reported to the medical health professional and seek a medical help urgently. Excessive levels of benzyl penicillin sodium can be corrected by hemodialysis.
